County Faculty Appraisal Forms - updated Nov 11, 2009

• County Faculty Self-appraisal Form (MS Word .doc )
  To be used by all county faculty for self-appraisals
• 4H RSA Appraisal Form (MS Word.doc)
  To be used by all in appraisal of the 4H RSA component 
• CED Appraisal Form (MS Word .doc)
  To be used by county faculty for appraisals of their County Extension Director and by County Extension Directors for self-appraisals. All CED appraisals are sent to the DED
• County Faculty Appraisal Form by CEDs & DEDs (MS Word .doc )
  To be used by District Extension Directors and County Extension Directors for county faculty appraisals.

  General Recommendations that all DEDs will be asking County Faculty for in 2009:

  1.Develop and write a minimum of two to three “measurable objectives” for each of your program areas

  2.Provide to the DED instruments of written evaluations of one of your program areas

  Report on all:
  Program Outcomes (i.e. knowledge gained and behavior changes),
  Impacts (i.e.  economic, social and environmental )
  and
  Success stories that are consistent with your Measurable Objectives from your program areas

• CED Appraisal Form for County Administrator (MS Word .doc)
  To be used by District Extension Directors only
