Semester Faculty Assignment Report
This is to inform you that the Semester Faculty Assignments for the Fall 2008 semester are due July 25th , to your District Director. The Fall Semester dates are Aug 8, 2008 to Dec 25, 2008
Overview
All County faculty are required to fill out the modified Semester Faculty Assignment Report form prior to each semester's work. Information from this form will then be utilized to place individual faculty members on Smith-Lever funds to meet our required multi-state and integrated activities for AREERA.
County faculty should fill out and sign paper copies of this form (original only), give it to their CED to initial, and then send it to their DED. This must be done each school semester. Please note: At this time, all forms must come to the DED as original hard copy.

Instructions
Each agent should download the file from above. While you have the form on your screen, you can select the "Text select Tool" to fill out the form on your screen or print and follow directions below.
Faculty only need fill in the highlighted areas, including:
- Name: ( your first and last name )
UF ID Number: ( not your SS number )
Semester: ( fall, winter, spring, summer )
Year: - Line 7a: ( the percentage for your general duties )
Line 7b: ( the percentage for your multi state activities
Line 11: ( check your percentage of time. most people are employed 100% )
County Extension faculty will be assigned 100% to the Cooperative Extension line. However, please note the section at the bottom of the form, where you should indicate which State Focus Team programs you are involved in, particularly from a multi-state approach. This information is very important.
In the box on the lower left side, fill in the appropriate State Focus Team number that your multi-state Extension activities will be reported in and the percentage of time that you spent on that activity..

Once you have completed your form, follow these steps.
- Print the completed form and sign in the signature area (lower right). We must have an original; no faxed copies will be accepted.
- Give the form to your CED for his or her initials and date (next to your signature line).
- You or your CED must make sure that the completed form is mailed to your DED before the due date for your work from on the next semester (listed above). Please note: At this time, all forms must come to the DED as original hard copy.
Definitions for Documenting Multi-state Activities
Multi-state activities means collaborative efforts that reflect the programs of institutions located in at least two or more states or territories. Multi-state activities may be carried out in person, on the phone, by U.S. post office, email, etc. In other words, contact does not have to be one-on-one.
Overall objectives may be the same (e.g., "weed control in cotton" or "nutrition education to reduce diet-related diseases"), while specific objectives may differ (e.g., Florida is more interested in working on mechanical weed control in cotton or diabetes education, while Georgia wants to work on chemical weed control in cotton or nutrition as it relates to heart disease). These are just examples; work with your DED if you have questions.
Funding for multi-state activities can be as simple as a person’s own salary or may involve grants and other forms of outside funding. Funding does not have to involve any transfer of money between states.

In the box at the bottom of the form, program/project numbers and percentage amounts are requested for multi-state and integrated activities. Can you provide further clarification as to whether an agent would put 0% or leave the box blank?

Leave blank any items that agents are not working on. We do have all types of agents working with specialists on integrated and multi-state projects.
